Transmission mechanisms of antimicrobial resistance and bacterial strains
Resistance of bacteria against antibiotics is rising to dangerously high levels in all parts of the world. New resistance mechanisms are emerging and spreading globally. Drugs that were once lifesavers are now useless and the treatment of many infectious diseases is becoming harder, and sometimes even impossible. With a lack in the development of novel antibiotics, the conservation of existing ones is crucial. Using molecular epidemiological approaches and several molecular typing and sequencing technologies, we study the emergence and spread of antimicrobial resistant (AMR) microorganisms and their mobile genetic elements. Such surveillance of AMR is of crucial importance in order to detect resistant strains of public health importance at an early stage and support the timely notification and investigation of outbreaks.

Researchlijn seksueel overdraagbare infectieziekten (als onderdeel van Public Health)
Op het gebied van de seksueel overdraagbare infectieziekten heeft de onderzoeksgroep een belangrijke internationale positie gerealiseerd. De focus ligt hierbij op het continuüm tussen microbiologie, epidemiologie en gedrag om daarmee bij te dragen aan een optimale soa-bestrijding in binnen- en buitenland. Dit is gerealiseerd door het uitbouwen van een aantal kernkwaliteiten die binnen de groep aanwezig zijn:
1. Unieke brug tussen publieke gezondheid (GGD) - met grote soapoli (>7000 patiënten per jaar) met systematische registratie – en innovatieve medische microbiologie, het onderzoek valt binnen het onderzoeksthema ‘Infectious Diseases and Antibiotic Resistance’ van de Academische Werkplaats Publieke Gezondheid Limburg waarin gewerkt wordt aan de versterking van de relatie tussen onderzoek, praktijk en beleid om door innovatie kwaliteit te blijven garanderen in een steeds veranderende omgeving;
2. Methodologische diversiteit (o.a. klassieke epidemiologie, key population benadering, netwerkmethodologie, geografische informatie systemen, e-health aanpak, web-based respondent-driven sampling);
3. Microbiologische innovatie. Er zijn in de afgelopen periode innovatieve diagnostische en onderzoeksmethoden ontwikkeld zoals o.a. de viability-PCR, kweek van Chlamydia trachomatis, resistentiebepaling bij C. trachomatis, PCR-directed kweek van Neisseria gonorrhoeae, kweekvrije typering van Neisseria gonorrhoeae (NG-MAST) en serologie op bloedspots. Tevens is er een start gemaakt met verdere ontwikkelingen op het gebied van high-throughput typeringstechnieken en moleculaire resistentiebepaling van diverse SOA-gerelateerde pathogenen;
4. Grote biobank met meer dan 100.000 monsters;
5. Relatie met gedragswetenschappers (o.a. stigma, intervention mapping strategieën).De multidisciplinaire aanpak wordt vorm gegeven binnen research school CAPHRI (Care and Public Health Research Instituut) en onderzoekslijn Health, Inequity and Societal Participation. Naar de toekomst zal de internationale positie verder worden voortgezet en versterkt. Daarbij zal meer aandacht komen voor internationale samenwerking en uitbreiding van het netwerk om daarmee kansen in toekomstige subsidieaanvragen te vergroten en de impact van de output van het onderzoek en daarmee de positie van de MUMC+/GGDZL tandem te versterken. Behalve wetenschappelijke impact heeft de soa research groep ook nadrukkelijk impact gehad op maatschappelijk vlak met veel media aandacht naar aanleiding van verschillende publicaties, verbetering van richtlijnen en herziening van nationaal en internationaal beleid. Dankzij de succesvolle studies op het gebied van Chlamydia trachomatis is vanaf 2014 het Nationaal Chlamydia trachomatis Referentie Laboratorium naar het MUMC+ gehaald in samenwerking met Amsterdam UMC, locatie VUmc. Tevens worden vanuit de afdeling in samenwerking met QCMD (Schotland) al enige jaren internationale moleculaire diagnostiek kwaliteitsrondzendingen op het gebied van SOA verzorgd.

The human microbiome
The billions of microbes that colonize the human body are collectively referred to as the microbiome. Most microorganisms inhabit our gastrointestinal tract, but also the skin, urogenital tract, and the respiratory tract are colonized by a wide variety of microbes.
The human microbiome plays a key role in human health by providing a natural barrier against pathogen colonization, supporting the provision of nutrients and exerting a critical role in shaping the mucosal immune system. Moreover, metabolites and innate immune stimuli produced by enteric microbes have numerous effects beyond the site of microbial colonization. Understanding what defines a healthy microbiome is fundamental to maintain healthy living and prevent disease development.
Our department has a long track record on microbiome research that goes back to the early 1990’s, long before microbiome research became such a booming field of science. At present, microbiome research at our department is at the international forefront with on-site automated sample processing, next-generation sequencing facilities and extensive bioinformatic expertise. The microbiome research group has strong links with clinical departments within our MUMC+ and is involved in many national and international research consortia, including the ambitious Million Microbiomes of Humans Project.
To further strengthen our Euregional collaboration on microbiome research and cross borders between research disciplines, we recently initiated the Euregional Microbiome Center (microbiomecenter.eu) together with research groups at Maastricht University Campus Venlo, Uniklink RWTH Aachen and the University of Liège.
There are three main themes within the microbiome research group.
- Development of a healthy microbiome
In order to understand which dietary, environmental and host factors contribute to the development of a healthy microbiome, we study the microbiome establishment from birth onwards within the context of many population-based birth cohorts. By profiling the microbiome of thousands of babies, toddlers, children and adults, we aim to understand what drives the inter-individual variation in microbiota composition and function and how this relates to a healthy life. Next to these observational studies in humans, mechanistic in vivo and in vitro studies are conducted to understand the interactions between the microbiota, diet and the host.
This research line is embedded within large national and international consortia such as the NWO Carbohydrate Competence Center, the JPI Healthy Diet for a Healthy Life and the World Universities Network inVivo Planetary Health. Contract research with (international) companies as well collaboration with microbiome related institutions are part of our research program. - The microbiome in health, disease and routine patient care
Next to maintaining a healthy microbiome for a healthy life, the microbiome is a target for treatment of already manifested diseases by the application of functional food ingredients, viable bacteria (probiotics), fecal transplant strategies or novel drugs and constitutes of a pool of innovative non-invasive diagnostic and prognostic biomarkers. In close collaboration with many clinical departments within our MUMC+ as well as across the world, we examine the role of the microbiome and microbial components, such as membrane vesicles, in a wide variety of diseases, including chronic intestinal diseases (Inflammatory Bowel Disease, Irritable Bowel Syndrome), metabolic diseases (overweight and obesity, type 2 diabetes), chronic respiratory diseases (COPD, asthma) and several types of cancer. Automated rapid technologies and Artificial Intelligence algorithms are studied for clinical purpose and improvement of patient care, e.g. to predict and/or prevent exacerbations and optimize treatment in an early phase based on defined changes in the microbiome composition.
In recent years, it has become apparent that the microbiome also has a significant influence on treatment drugs as specific microbes can convert drugs into active, inactive or even toxic components. We therefore also study the impact of the microbiome on treatment response. - The microbiome as a reservoir of antimicrobial resistance
Although the extensive overuse and misuse of antibiotics in both humans and animals is mainly fueling AMR, it is the increased globalization that subsequently results in rapid dissemination of (novel) AMR bacteria or bacterial mobile genetic elements from AMR hot zones, such as South and Southeast Asia, to other parts of the world.
The intestinal microbiome is the most important reservoir through which these travelers to AMR hot zones may acquire and import AMR. The gastrointestinal tract is an open system, which every day encounters a myriad of bacterial acquisitions originating from the environment (e.g. from food, water, soil, and other humans or animals. These incoming bacteria acquired in countries with a high prevalence of AMR often harbor antibiotic resistance genes. Consequently, local emergence of AMR can rapidly become a worldwide health problem.
Within our department, we study the impact of migration (including tourists, business travelers, students and refugees) on the import and spread of AMR. We are amongst others coordinating the worldwide largest study on the Carriage Of Multidrug resistant Bacteria After Travel (COMBAT-study). Using a combination of metagenomic approaches, we mine the gut microbiome to reveal potential novel AMR genes. This is not only done in travelers, but also in local populations, the environment and livestock in countries with a high prevalence of AMR (e.g. India and Vietnam).
- Development of a healthy microbiome

In de grensstreek Vlaanderen-Nederland werken negen ziekenhuizen, zeven publieke gezondheidsdiensten, zeven kennisinstellingen en drie bedrijven samen aan het i-4-1-Health-project. Dit project richt zich op infectiepreventie en het bestrijden van antibioticaresistentie bij mens en dier (One Health) door middel van onderzoek en innovatie. Het project start op 1 januari 2017 en duurt tot en met 31 december 2019. Het Amphia Ziekenhuis is de projectverantwoordelijke organisatie en het Universitair Ziekenhuis Antwerpen is co-projectverantwoordelijk.

Rol van het academisch ziekenhuis Maastricht in dit project
De afdeling Medische Microbiologie van het academisch ziekenhuis Maastricht (onderdeel van het Maastricht UMC+) is werkpakketleider van Werkpakket 7 ‘Track en Trace van resistente Bacteriën (Whole Genome sequencing platform)’. WP7 focust op het in kaart brengen van de aanwezigheid en het volgen van de verspreiding van antimicrobiële resistentie en van de micro-organismen die deze resistentie dragen in de veehouderij, in de publieke gezondheidssector en in de zorg. De procedure omvat alle stappen van monstername en wet-lab analyses (door academisch ziekenhuis Maastricht, Universiteit Antwerpen en Universitair Ziekenhuis Antwerpen) tot bio-informatica-gestuurde interpretatie van de data.

Sinds de uitbraak in december 2019 heeft COVID-19 de hele wereld getroffen, inclusief Europa. Het heeft dus ook veel invloed gehad op de Euregio Maas-Rijn. Momenteel is de kennis over de verspreiding van de COVID-19 in de bevolking in relatie tot het wel of niet naleven van de infectiepreventiemaatregelen beperkt. Dit is echter wel de basis om de ziekte te kunnen monitoren en indien noodzakelijk maatregelen op te leggen of juist op te heffen. Dit project is ontwikkeld om gegevens te verzamelen over het voorkomen van COVID-19 en hoe dit zich verhoudt tot het gedrag van de bevolking in alle drie de landen van de EMR.

Het doel van het project is om de impact van COVID-19 op de EMR te beoordelen: door het voorkomen van COVID-19 antilichamen te relateren aan de genomen maatregelen, zodat inzicht wordt verkregen in de effecten van COVID-19 in een grensoverschrijdende regio als de EMR. Dit wordt ook gepresenteerd als een showcase voor andere grensregio's in de EU (of zelfs breder).
Activiteiten
- Er zullen in twee rondes bloedmonsters worden afgenomen en geanalyseerd om de ontwikkeling van COVID-19-antilichamen onder EMR-burgers te zien. Dit wordt in verband gebracht met de verschillende genomen maatregelen in de landen van de EMR.
- Er zullen regionale burgertops worden georganiseerd om zo ook de stem van de burger mee te nemen. Middels deze burgertops wil het project inzicht krijgen in hoe zij COVID-19 hebben ervaren.
- Er zal een conferentie worden georganiseerd waar de eerste resultaten van dit project zullen worden gepresenteerd en waar het rapport met de aanbeveling zal worden besproken met deskundigen uit de hele EMR en daarbuiten.
Resultaten
Naast het eindrapport welke de bevindingen van het onderzoek en de regionale burgertops omvat, zal een infographic worden gemaakt van het resultaat van het rapport voor een betere illustratie, en zullen alle gegevens die door het project zijn verkregen, worden gepresenteerd op de Euregional Health Atlas (EHA) (Euregionale Gezondheidsatlas).
